Web Form Templates

123ContactForm provides an evergrowing collection of web form templates. Currently, we have over 60 templates, structured by user category and by web form type. You can find the template that you’re looking for quickly. And it will be published in no time.

The templates collection contains lead generation forms, order forms, surveys, registration forms, and various other types of web forms. They are addressed to small businesses, designers and developers, event organizers, consultants, NPOs, and many other user groups.

Most web form templates can be used on the free plan. Advanced templates require being on a paid plan. The badge found at page top indicates the service plan that the template can be used on. If you want to see what each plan provides, have a look at our features matrix.

Templates can be customized easily. Our drag & drop editor allows you to make any changes you need quickly. To start using a web form template, follow the two very simple steps below:

1. Browse through the web form templates collection and, when you find the template that you need, click on the big green button Create a new form using this template. The template will be duplicated and added to your My Forms section.

2. After the form template has been duplicated, if you are logged in to your account, you will be taken straight to your Form Editor to customize the template. Otherwise, you will be asked to log in or sign up and then redirected to the Form Editor.

Later on, when you want to access your customized web form template, you’ll be able to find it in the My Forms section. Customized templates behave like any other one of your forms.

Enjoy! And send us your suggestions for new web form templates.

Here is a breakdown of the Settings → Notifications section. We know there are many settings there that can be adjusted, so let’s make them crystal clear.

Notification Emails

Where submissions are sent to. The email addresses listed here are the recipients of the messages sent through the form. You can either send all submissions to a certain email address or only certain submissions, based on user input. To add a new notification email, click on Add Notification Email. If you want to delete one of the email addresses, simply tick its checkbox and then click Delete Selected. Find out more about notification emails and custom recipients.

Confirmation Settings

Actions made after submission

First, you can choose whether, after form submission, users will see a thank-you message, will be redirected to a web page or whether both actions will be made. Then, you can choose to send a copy of the completed form to the sender. Read more about these two actions made after submissions.

Confirmation messages (autoresponders)

Confirmation messages (autoresponders) are custom messages sent to form users after submission. You can use the same autoresponder for all users or create several autoresponders and send them based on user input. Use the Manage button to create and edit confirmation messages and go to Advanced Confirmation Rules to establish which autoresponder is sent in which case.

Send Confirmation Message To should be set on the Email field where the user will fill in the email address to which you want to have the autoresponder sent.

Confirmation Message Sender Email is the email address that will appear as the sender in the confirmation message that the user will receive.

Confirmation Message Sender Name is the name that will appear as the sender in the confirmation message that the user will receive.

Customize Emails

Email Template defines the way submissions look when they arrive in your inbox. By default, submissions are structured in rows, grey and white alternatively, each containing one field (label and input). You can change the structure of this email by modifying email templates.

A form that is integrated with your Top Producer account can send information about the lead type that is generated at form submission. To achieve this, you will need to add a new field to your form and set a default value for it. After matching 123ContactForm fields with Top Producer fields, this value will be passed to your Top Producer list. Follow the step-by-step guide below to set this up.

Add a new field to your form. Label it anyway you want. For example purposes, we will call it Lead Type.

Set a default value for the new field in your Form Editor. The values that can be set for this field are: Buy-New-Home, Buy-Resale-Home, Buy-New-Resale-Home, Sell Home, Rent.

Note: This value is the Role that can be specified for your Top Producer leads. When neither of the values above is passed to Top Producer, the default value, Buy-New-Home, is used.

Hide the field by ticking the checkbox Hidden in the Edit Field panel. This step is optional, but you probably don’t want form users to see the form field that will pass the lead type.

You are now done with the Form Editor. Next, go to your Settings → Applications section and enable the Top Producer integration, if you haven’t done so yet.

When matching fields, make sure to associate the Top Producer field labeled Role with the 123ContactForm field that you have just added, Lead Type. Press Save and you’re good to go.

When a new form submission is made and information is passed from your 123ContactForm web form to your Top Producer list, the role of the new lead will be the one specified in your Lead Type field. You can change this value and publish the form on different pages with different values (or share the form link using different values).

Sometimes, the input provided in two form fields must be identical; for example, when you are asking users to create a password or to provide and confirm their email addresses. You can enable this type of validation in the Form Editor.

First, add the two fields in which the input should be identical. Then, click on one of them to open its Edit Field panel. Go to Advanced Options and tick the checkbox Match with the value of another field. In the dropdown list that appears, select the matching form field.

When you do this, the other field is also paired with the first one, in its own Advanced Options section.

In the live version of the form, if users provide different field values for the two fields, they get an error indicating that the input must be identical. The form can’t be sent otherwise.

Note: If the two fields are placed on different pages, then the matching is ignored.
