This form is a request and does not guarantee that the facility is available for the date(s) or described event. Applicant will be notified if your date(s) or event is accepted under the conditions specified. Notification will be written letter or electronic response. Expect a personal interview before final application is accepted. Financial arrangement will be indicated: deposit, rental fees, janitorial, and all others.
