Dear Parent or Legal Guardian,
Thank you for allowing your child to participate in a So Sadity Event. Please complete the form below to consent that your child has permission to participate in all activities during the event. By signing this consent you are acknowledging and agreeing that So Sadity LLC, nor the employees or affiliates of So Sadity LLC, will be held liable for any accidents, incidents or allergic reactions (to food, beverages, clothing, or make up, etc.) provided by So Sadity, LLC during or after the party.
FYI: It is best to have your child dressed in biker shorts and camis or items similar to easily slip costumes and tutus over. Costumes, tutus, and or robes are provided for use during each party. Nail Polish Remover will not be used; Bare Nails are best.
