Student Medical Release, Authoriziation & Consent by Parents

Bonjour Chicago and Séjours HOME ABROAD may without liability or expense to themselves take whatever action they deem appropriate with regard our child’s health and safety.

Bonjour Chicago and Séjours HOME ABROAD may place our child in a hospital for medical service and treatment, or, if no hospital is readily available, to have our child treated by a local medical doctor. If deemed desirable, by Bonjour Chicago Séjours HOME ABROAD, we (parents) authorize them to transport our child back to our country of residence at ours (parents) expense.

I hereby grant my full permission and consent for the temporary guardian to establish a place of residency for my child to reside and travel with said temporary guardian.

I hereby grant the temporary guardian my full authorization to make all decisions related to my child's educational and recreational activities and undertakings during their stay in the U.S.A.

We (parents) accept full responsibility for all medical expenses and/or transport for our child either by our insurance company or by ourselves.

Student Guidelines, non-smoking, and Conduct Agreement

Smoking in the USA and Canada is restricted by law. Students participating in an American Homestay program agree that they will not smoke while in the USA.  If they do so, they will be returned to their country at the expense of their parents.

Always be polite to your host family and to the people you meet.  You are an ambassador for your country and the impression you leave depends on your attitude.

Attend all classes and activities when scheduled.  Your attendance is important and mandatory. Only if you are ill or have special permission from your host parents should you be out of class.

Tidy up after yourself in your host families home.  Keep your room neat, make your own bed, and do you own laundry.  Always offer to help with household chores and family errands. This is an everyday part of family life, and it is important to you to be part of the family, and not always a guest.  It also a sign of respect.

Ask host parents’ permission before inviting friends home or to a meal. Also ask to use the telephone for long distance calls. Be sure to pay for long distance calls immediately (before making the call, as the operator for time and charges), or use a prepaid phone card, or collect call. Any phone bill born by the host family for your
calls will have to be reimbursed by your parents.

Always offer to pay your own way when out to dinner or at a movie or outing. Your family may not always want you to pay, but be sure to offer every time.

Remember, you will not be allowed to operate an automobile or motorcycle while in the United States or Canada. Drinking alcoholic beverages without the consent of the host family is not allowed.

Inform your family where you are going, whom you are going with and when you will return. This is done out of courtesy and respect for your host parents and their responsibility for you. Always be sure to be very accurate with this information. If you think you will be late, call and let your parents know.

Participate in all aspects of family life. Your host family wants to learn from you as much as you want to learn from them.
