Executive Master Black Belt Program Admission Form


The Dr. Mikel J. Harry Six Sigma Management Institute, 3370 North Hayden Road, Suite 123-320, Scottsdale, AZ; herein referred to as SSMI, and the Executive Master Black BeltTM program applicant, herein referred to as the STUDENT.  The Executive Master Black BeltTM training and certification program is herein referred to as the EMBB PROGRAM.


1) Timing and Venue.  The current EMBB PROGRAM start date is published at http://www.executivembb.com.  The EMBB PROGRAM consists of five (5) days of live classroom activity and followed by ten (10) days of supported independent laboratory activity. The classroom venue for the live portion of the EMBB PROGRAM will be solely determined by SSMI.  The student will be notified of the classroom venue via electronic means at least ten (10) business days prior to the published EMBB PROGRAM start date.  The laboratory portion of the EMBB PROGRAM must be completed within thirty (30) business days of the EMBB PROGRAM start date; otherwise, the STUDENT will be disqualified from the EMBB PROGRAM.  In this event, the STUDENT is not entitled to any type or form of tuition refund or reimbursement from SSMI.

2) Tuition and Payment.  The standard EMBB PROGRAM tuition is $16,500 per STUDENT. However, if a STUDENT’s EMBB PROGRAM tuition payment is sent to SSMI at least twenty (20) business days prior to the published EMBB PROGRAM class start date, the STUDENT’s tuition will be reduced to $15,000.   The EMBB PROGRAM tuition due date is defined as ten (10) business days following the STUDENT’s official EMBB PROGRAM application acceptance date.  The EMBB PROGRAM late application date is defined as ten (10) business days prior to the EMBB PROGRAM class start date.  In the event of late application, the STUDENT will be assessed a five percent (5%) surcharge.  The following forms of tuition payment are accepted by SSMI: 1) Certified Cashier Check; 2) Bank Wire Transfer; and 3) Automated Clearing House Transfer (ACH).  In the event of late payment, the STUDENT will be assessed a five percent (5%) surcharge.  Regardless, the STUDENT’s tuition payment must be received by SSMI not later than the EMBB PROGRAM class start date.
 
3) EMBB Class Cancelation.  In the event a STUDENT elects to cancel participation in the published EMBB PROGRAM, the STUDENT must provide SSMI written electronic notice of such cancellation.  If the effective date of the STUDENT’s written cancelation is less than twenty (20) business days prior to the published EMBB PROGRAM class start date, the STUDENT is entitled to participate in the next available EMBB PROGRAM.   If the effective date of the STUDENT’s cancelation is greater than (20) business days prior to the published EMBB PROGRAM class start date, the STUDENT is entitled to a fifty percent (50%) tuition refund or may elect to participate in the next available EMBB PROGRAM.  If the published EMBB PROGRAM is canceled due to Force Majeure, or SSMI elects to exercise its acknowledged right to cancel the published EMBB PROGRAM, the STUDENT will be entitled to a full tuition refund.  SSMI does not warrant or guarantee there will be another EMBB PROGRAM scheduled beyond the current published EMBB PROGRAM class start date.

4) Intellectual Property.  Ownership of all copyright and other intellectual property rights in the SSMI Authorized Course Material, including any documentation, data, technical information and know-how provided to the STUDENT as part of the EMBB PROGRAM remains vested in the owner of the Course Materials.  The STUDENT further agrees that all such information shall be held in strict confidence by the STUDENT and may not be copied, disclosed, divulged, transmitted or made known to any third party (including SSMI business competitors) without the express written permission of SSMI or its rightful owner.

5) Non-Duplication.  The STUDENT agrees not duplicate or create a full or partial facsimile or likeness of the EMBB course design, content, outline, curriculum, tests, exams, quizzes, objectives, intents, terminology, processes, guidlines, proceedures or practices that could or might be used to comercially or nocomercially compete with the EMBB Program.  The STUDENT also agrees not to cause or otherwise bring about any type or form of change, alteration, enhancement, modification, addition, deletion, and/or adjustment (in any manner) to any SSMI intellectual property without the written permission of SSMI.

6) Program Performance.  The STUDENT agrees that, at all times; and under all circumstances, SSMI retains the unrevocable right to establish, alter, change or otherwise modify any or all EMBB program admission criteria, instructional content, test questions, test answers and certification requirements for any reason SSMI deems necessary; and done so without the prior notification of such to the STUDENT or the STUDENT's employer or sponsoring organization.  The STUDENT also agrees that SSMI retains the right to fully and unilaterally enforce the most current program certification criteria.  The STUDENT further agrees that any and all decisions, judgements and/or determinations rendered by SSMI, as related to the STUDENT's unelective discontinuance
in the training and certification program, is done so without appeal or recourse by the STUDENT.  In addition, the STUDENT understands and agrees that SSMI retains the right to terminate (at any time) the STUDENT's participation in the training and certification program based on SSMI's judgements and decisions concerning the STUDENT's compliance to the program certification criteria and/or the STUDENT's training performance.  The STUDENT further agrees that the STUDENT's employer, or sponsoring organization, may require the planning and execution of one or more on-the-job project(s) as a portion of the STUDENT's certification or as a part of the STUDENT's normal work duties.  The STUDENT also agrees that SSMI does not warrant, guarantee or promise the STUDENT any type or form of personal or professional benefits as a result of successfully completing the training and certification program.

    Step 2: Dr. Harry reviews your application submission form and relevant sources.

    Step 3: Dr. Harry will notify you of your application status via email.

    Step 4: Identify and secure your source of tuition funding.

    Step 5: Email Don Drury your tuition payment information.*

    Step 6: Don Drury will contact you regarding your pending enrollment.

    Step 7: Sandra Harry will send you the tuition invoice for dispositioning.

    Step 8: Don Drury will contact you upon tuition payment and include an Orientation Package.

Section 7: Black Belt Competency Self-Appraisal
For this portion of your admission application, you will be asked to rate your competency level for 10 different common categoires of statistical tools and methods.  To help you with this, the following link provides a list of the specific tools and methods used by the Six Sigma Managment Insitute to train and certify Black Belts.  Thus, this list is a guideline that specifies the tools and methods you will need to have a working knowledge of in order to successfully participate and complete the Executive Master Black Belt program of study.  For example, one of the below listed categoies is called "Inferential Statistics"  You might be wondering about the scope and depth of knowledge that is associated with this category.  To check on this, simply click on the following link and look-up the "Inferential Statistics Module."  Once you read through this list and the instructional objectives, you will have a much better idea of how to rate your level of competence where "Inferential Statistics" is concerned.

Section 8: Voluntary Personality Inventory
The 10 Item Personality Indicator is not a required for admission into the EMBB program of study.  However, we would gratefully appreciate you taking a few minutes to fill it out and post your scores into this application form (see below).  Again, we do not use the scores (in any way) to evaluate your worthiness for acceptance; or as a set of decision criteria, but rather we look at the scores so that we may "get to know you a little."  In other words, the survey results give us a snapshot of the man or woman behind the resume.  Thanks in advance for voluntarily completing this portion of the EMBB application.
