Disclosure Agreement
I allow this information, including medical documentation, to be reviewed by the Housing Accommodation Committee
in order to make a determination regarding my request to be considered for an on-campus housing accommodation.
Your file will only be reviewed by University of Mobile administrators in offices relevant to your case. These offices may include the Disability Support Services Office, Residential Life Office, Health and Counseling Services and Dining Services if applicable.
The Housing Accommodation Committee will consider your requests and utilize these materials to determine the most appropriate placement and/or accommodation for you.
Once a determination of eligibility placement and/or accommodation is made, you will be notified of your accommodations by the Disability Support Services Office. The Residential Life Office will notify you of your specific placement or relevant administrative information for your accommodations.
Students who decline their placement and choose to go through the general room selection process will not be able to retain the original accommodated assignment once declined.
Submission of the Housing Accommodation Request Form, with or without supporting medical documentation, does not guarantee that you will receive the requested accommodation. University of Mobile reserves the right to propose alternative reasonable accommodations based on an individual assessment of your case.
