All aerial activity participants are required to complete and sign an e-Waiver Form. A parent or legal guardian must complete the form if the participant is below the age of 18 years old.

You can fill one out at the studio, or you can save time as early as now by submitting online.

Early online submission is most ideal if a child below the age of 18 years old is carpooling to our facility without their parent or legal guardian.
